Issuance of Certificate of Tree Plantation Ownership (CTPO)

Requirements

1. Filled-out Online Application Form or Letter of Application/Intent
2. Authenticated copy of ownership to the Land
a. Original Land Title, Transfer Certificate of Title, Certificate of Land Ownership Award or Tax Declaration of Untitled Alienable and Disposable (A&D) Lands
3. Certificate of tree plantation ownership from the Barangay Captain or Executive of the Municipality/City
4. Picture of the Tree Plantation
5. Authorization Letter from the co-owner (in case the applicant/tree farmer is not the sole owner of the land)
